Ducts in the human nipple: structure, epithelial immunophenotypes, and relationships with clear cells of nipple epidermis (toker cells)

چکیده
It has been suggested that there are structurally distinct 'type A' and 'type B' ducts in human nipple. If so, properties such as their relative numbers, accessibility at the nipple surface, and relationships with the breast lobes they communicate with could have biological significance and practical relevance for the intraductal approach to breast cancer diagnosis and treatment. To investigate the two-duct-type hypothesis further we examined duct structure and duct epithelial immunophenotypes in the nipples of cancer mastectomies. Relationships between major nipple ducts and clear cells of nipple epidermis (Toker cells) were also explored. Sections through the nipple duct bundle at the base of the papilla in 10 mastectomy nipples were immunostained for oestrogen and progesterone receptor; proliferation markers Ki-67 and mcm-3; Cox-2; E-cadherin; basal epi-thelial markers p63, CD10, keratins 5, 14, 17 and luminal epithelial markers keratins 7/8 and 19. In 30 further cases sagittal (and some coronal) sections were immunostained for keratins 7/8, 14 and progesterone receptor. Duct perimeters were measured for all ducts in transverse sections of the nipple duct bundle in 15 cases. We did not find unambiguous evidence supporting the two-duct-type hypothesis. Duct perimeters at the base of the papilla were variable but did not apparently define separate populations of larger and smaller ducts; nor could separate duct populations be recognised on their epithelial immunophenotype. However, while some ducts open directly into a funnel-shaped infundibulum, others apparently taper to a pinpoint lumen, and may cluster around a common opening. Clear cells of nipple epidermis (Toker cells) were present in variable numbers in 31/40 cases, usually around duct openings on the nipple surface, sometimes show features suggesting possible locomotion, and are occasionally present in keratin plugs. Similarities between clear cells of nipple epidermis and mammary precursor cells have been noted previously; their location and possible motility suggest new avenues for research.

We report herein a 41-year-old woman who underwent excisional biopsy of a firm mass beneath the left nipple. Gross examination of the lesions revealed a firm irregularly-bordered cream-gray mass measuring 1.5×1.5×1cm which histologically exhibited an infiltrative tumor constituted by small cord-like and angulated tubules individually set in an abundant fibrous stroma. These tubular ...
متن کاملAn overview of the pale and clear cells of the nipple epidermis.
The stratified squamous epithelium of the nipple-areola complex may contain pale or clear cells including: Paget's disease cells (PDCs), Toker cells (TCs), and so-called clear cells (CCs). Paget's disease is an uncommon presentation of breast carcinoma. PDCs are large, atypical, have abundant, pale-staining cytoplasm that may contain mucin secretion vacuoles and bulky heterochromatic nuclei. Th...

A 62-year-old woman was referred by her GP to the West Sussex Breast Unit with a pigmented lesion on her left nipple that had been increasing in size for six months. She had recently moved back to the UK after living in Australia. She did report recent bleeding from the nipple that had resolved completely. She had no previous breast disease and there was no family history of breast or ovarian c...
متن کاملExtramammary Paget’s Disease Covered the Left Nipple and Areola
We present a case of a 71-year-old woman suffering from mammary Paget’s disease and having a 10-years history of an irregular, widespread erosion accompanied by itching and burning on the skin of her left chest, extending to the breast. The erosion had steadily enlarged and had become increasingly tender. The nipple and areola of the left breast disappeared and could not be recognized. No abnor...
متن کاملNipple adenoma arising from axillary accessory breast: a case report
UNLABELLED Nipple adenoma is a relatively rare benign breast neoplasm, and cases of the disease arising from the axillary accessory breast have very seldom been reported in the English literature. We report a case of nipple adenoma arising from axillary accessory breast including clinical and pathological findings. An 82-year-old woman presented with the complaint of a small painful mass in the...
